OFW Services and Procedure

Registration of  Name Hire
(Skilled Workers/Professionals)
NAME-HIRE UNIT, 2ND FLOOR

SERVICES:

  • Evaluation and registration of employment documents

  • Referral for medical examination and Pre-Departure Orientation Seminar

  • Issuance of e-Receipt/Exit Clearance

CLIENTS/CUSTOMERS:

Workers with employment contract obtained from foreign employer without the assistance or participation of any recruitment agency

 SERVICE FEE:

Phase I

     NONE

 Phase II

  • Processing Fee                   -   US$100.00 or peso equivalent

  • OWWA Membership Fee     -   US$25 or peso equivalent

  • Phil-Health Coverage          -   PhP900.00

DOCUMENTARY REQUIREMENTS

Phase I

  •  Employment Contract duly signed by the employer and worker

  • For female low-skilled workers - employment contract duly signed by the employer and worker verified by Philippine Overseas Labor Office

  • Valid working visa/work permit/no objection certificate (NOC) or visa assurance from the employer or any equivalent entry document

  • Valid passport (original and photocopy)

  • One (1) piece 2x2 picture (for medical referral)

Phase II

  •  All documents listed in Phase 1

  • OFW Information Sheet

  • Pre-Departure Orientation Seminar (PDOS) Certificate of Attendance

  • Medical Examination result (fit to work overseas)

  • Affidavit of Declaration as a Name Hire, as necessary

  • Compliance Form, as necessary

PROCEDURE

Phase I

  • Submit documentary requirements for evaluation

  • Secure PDOS referral form and attend Pre-Departure Orientation Seminar

  • Secure medical referral form and undergo Medical Examination by DOH-accredited medical clinic/hospital

Phase II

  • Re-submit documents that were evaluated in Phase I, PDOS Certificate of Attendance, Medical Examination result and other documents as necessary

  • Secure assessment of fees

  • Pay fees and secure e-Receipt/Exit Clearance

ESTIMATED PROCESSING TIME

      Phase I: 30 minutes, Phase II: 2 hours
